Some people with long-term complex health needs, qualify for free social care funding. They are arranged solely by the NHS. This is known as NHS Continuing Health Care (CHC).
Am I eligible?
To be eligible for NHS CHC, you or your loved one must be assessed by a team of healthcare professionals, sometimes called a Multidisciplinary Team (MDT). The team will look at all your care needs and relate them to the following:
What do you or your loved one need?
How complex are the needs?
How intense can the needs be?
How unpredictable they are? Including any risks to health if the right care package is not available.
The person being assessed should be fully involved in the process and kept informed. Have all views about the needs and support taken into account. Carers and family members should also be consulted where appropriate.
A decision about eligibility for a full assessment for NHS CHC should usually be made within 28 days of an initial assessment.
Decisions made can be appealed. But there has to be very good evidence for appeal.
